POLICIES AND PAPERWORK

For the safety of our clients, patients, and staff, we do not permit unrestrained pets on our property. ALL pets must be on a leash or contained in a pet carrier. The front desk receptionist has extra slip leashes for your convenience. A cardboard pet carrier can also be provided for an additional cost.  If you need a leash or pet carrier, please see our front desk staff before getting your pet out of the car.

For the safety of all animals visiting the practice, we ask that your pet be up-to-date on vaccinations before using our boarding and surgical services.

We do provide a separate isolation ward for special circumstances at an additional cost.

We require payment at time of service.

We accept cash, check, debit card, Discover, Mastercard, and Visa.

Animal Medical Center will refill prescriptions on request provided our veterinarians have recently examined your pet and the particular medication or product is appropriate based on the veterinarian’s diagnosis.  The time frame of “recent examination” is at the doctor’s discretion, but will never exceed a maximum of one year from your pet’s last annual examination.

If you require a refill of your pet’s medication, we recommend you contact the office by telephone first to confirm we have the medications in stock and to give the hospital enough time for the staff to verify the refill is authorized by the veterinarian and prepare the refill.

Prescription requests before 11:00 am will be prepared and ready for pick up after 2:00 pm the same day.   Prescriptions requests after 11:00 am will be ready for pick up the following business day.  

We will not provide prescription refills for animals that are not current patients of our animal hospital, nor provide refills for animals that are patients of another animal hospital-even when those prescriptions are written by other veterinarians or veterinary hospitals.

Click below to access our Medical Records Release Form.  It is our policy that this form be completed and signed before your pet’s medical records can be sent to ANY other party (excluding medical referrals).  If your pet will visit a boarder, groomer, daycare, dog park etc….please print this form, sign, and fax or email for us to keep on file.
